Terms and Conditions

Effective Date: August 20, 2026

1. Acceptance of These Terms

Welcome to the website of Illume Anesthesia PLLC (“Illume Anesthesia,” “we,” “us,” or “our”), a provider-owned anesthesia group headquartered at 5700 Tennyson Parkway, Suite 300, Plano, Texas 75024, serving surgical practices, ambulatory surgery centers, and endoscopy centers throughout the Dallas–Fort Worth metroplex. By accessing or using illumeanesthesia.com (the “Site”), you agree to be bound by these Terms and Conditions and our Privacy Policy. If you do not agree, please do not use the Site.

2. About Our Services

Illume Anesthesia provides clinical anesthesia services — including GI and endoscopy anesthesia, office-based anesthesia, and ambulatory surgery center anesthesia — as well as anesthesia staffing and management services for healthcare facilities. The Site describes these services and allows prospective facility partners and patients to contact us or schedule a discovery call. Information on the Site is provided for general informational purposes only and may be updated or changed at any time without notice.

3. No Medical Advice; No Provider–Patient Relationship

Content on this Site — including descriptions of anesthesia techniques, pre-procedure preparation guidance, and answers to common questions — is educational in nature and is not medical advice. Your use of the Site, submission of a contact form, or scheduling of a discovery call does not create a physician–patient or provider–patient relationship with Illume Anesthesia or any of its physicians, CRNAs, or other clinicians. A provider–patient relationship is established only through a formal pre-anesthesia evaluation conducted in connection with a scheduled procedure. Always consult your physician or qualified healthcare provider regarding your specific medical condition, medications, and preparation for anesthesia.

4. Medical Emergencies

Do not use this Site to communicate about a medical emergency. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. The Site and our contact channels are not monitored continuously and are not appropriate for urgent clinical matters.

5. Communications and Scheduling

When you submit a form, request a discovery call, or otherwise provide your contact information, you consent to Illume Anesthesia contacting you by phone, email, or text message about your inquiry. Standard message and data rates may apply to text messages, and you may opt out of non-essential communications at any time. Please do not include sensitive health information in website forms or unsecured email; detailed clinical information should be shared only through the secure channels we establish with you or your facility.

Text Messaging (SMS) Terms

By providing your phone number through a form on this website, by calling us, or by otherwise opting in, you consent to receive text (SMS) messages from Illume Anesthesia PLLC, including responses to your inquiry, scheduling and appointment communications, and service-related information. Consent to receive text messages is not a condition of receiving any service.

  • Message frequency varies.
  • Message and data rates may apply.
  • Reply STOP at any time to opt out; you will receive one final message confirming your opt-out.
  • Reply HELP for assistance, or contact us at info@illumeanesthesia.com or 214-216-7527.
  • Wireless carriers are not liable for delayed or undelivered messages.
  • No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with, or sold to, any third parties.

6. Privacy and Protected Health Information

Our collection and use of information submitted through the Site is governed by our Privacy Policy. Protected health information we receive in the course of providing anesthesia care is handled in accordance with the Health Insurance Portability and Accountability Act (HIPAA), the Texas Medical Records Privacy Act, and our Notice of Privacy Practices, which is provided to patients in connection with their care.

7. Intellectual Property

The Site and its contents — including text, graphics, logos, the Illume Anesthesia name and mark, photographs, and page design — are owned by or licensed to Illume Anesthesia PLLC and are protected by copyright, trademark, and other intellectual property laws. You may view, download, and print pages for your personal, non-commercial use. You may not reproduce, distribute, modify, or create derivative works from Site content, or use our name or marks, without our prior written consent.

8. Acceptable Use

You agree not to use the Site to: (a) violate any applicable law or regulation; (b) submit false, misleading, or fraudulent information; (c) interfere with the Site’s operation or security, including through automated scraping, probing, or the introduction of malicious code; (d) impersonate any person or entity; or (e) infringe the rights of Illume Anesthesia or any third party. We may suspend or terminate access to the Site for any user who violates these Terms.

9. Third-Party Websites

The Site may contain links to third-party websites, including scheduling platforms, facility partners, or professional resources. These links are provided for convenience only. We do not control and are not responsible for the content, privacy practices, or availability of third-party sites, and a link does not constitute an endorsement.

10. Insurance, Billing, and Cost Information

Any information on the Site regarding insurance participation, billing practices, or estimated costs is general in nature, subject to change, and not a guarantee of coverage or a binding quote. Your actual financial responsibility depends on your insurance plan, the facility where your procedure is performed, and the services actually rendered. Patients are entitled to good-faith estimates and balance-billing protections as provided under applicable federal and Texas law; please contact us with questions about anesthesia billing for your procedure.

11. Disclaimer of Warranties

THE SITE AND ITS CONTENT ARE PROVIDED “AS IS” AND “AS AVAILABLE,” WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, NON-INFRINGEMENT, ACCURACY, OR UNINTERRUPTED AVAILABILITY. THIS DISCLAIMER APPLIES TO THE SITE ONLY AND DOES NOT LIMIT OR MODIFY ANY DUTY OWED TO PATIENTS IN THE COURSE OF ACTUAL CLINICAL CARE.

12. Limitation of Liability

TO THE FULLEST EXTENT PERMITTED BY LAW, ILLUME ANESTHESIA PLLC AND ITS OWNERS, PHYSICIANS, CRNAS, EMPLOYEES, AND AGENTS SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING OUT OF OR RELATING TO YOUR USE OF, OR INABILITY TO USE, THE SITE. NOTHING IN THESE TERMS LIMITS LIABILITY FOR PROFESSIONAL SERVICES ACTUALLY RENDERED TO A PATIENT, WHICH ARE GOVERNED BY APPLICABLE TEXAS LAW.

13. Indemnification

You agree to indemnify and hold harmless Illume Anesthesia PLLC and its owners, clinicians, employees, and agents from any claims, damages, or expenses (including reasonable attorneys’ fees) arising out of your violation of these Terms or your misuse of the Site.

14. Governing Law and Venue

These Terms are governed by the laws of the State of Texas, without regard to conflict-of-law principles. Any dispute arising out of or relating to these Terms or the Site shall be brought exclusively in the state or federal courts located in Collin County, Texas, and you consent to the personal jurisdiction of those courts.

15. Changes to These Terms

We may revise these Terms from time to time. The “Effective Date” above reflects the most recent revision. Your continued use of the Site after changes are posted constitutes acceptance of the revised Terms.

16. Severability and Entire Agreement

If any provision of these Terms is held unenforceable, the remaining provisions will remain in full force and effect. These Terms, together with our Privacy Policy, constitute the entire agreement between you and Illume Anesthesia regarding use of the Site.
